Take Charge of Your Health – Chronic Disease Self-Management Program

Date: Tuesdays – October 18, 25, November 1, 8, 15 and 22
Time: 10 a.m. – 12:30 p.m.

In the six-week Chronic Disease Self-Management Program, participants with chronic conditions such as high blood pressure, asthma, heart disease, and diabetes will learn new ways to take charge of their health and symptoms including how to address fatigue, pain, sleep, shortness of breath, stress, and emotional difficulties.  Additional topics include breathing techniques, falls prevention, healthy eating, physical activity, communication skills.
