Care Management

PCA provides high-quality care management for the OPTIONS program, which helps older adults age in place. Care management ensures that individuals receive the support they need to navigate the challenges of daily living. These support services are designed to address the unique needs of each person in their home, helping to prevent unnecessary hospitalization and institutional care. PCA’s care managers assist individuals in coordinating home and community-based services, allowing them to age gracefully in their homes.
