PCA Long Term Care Access (LTCA)
PCA’s Long Term Care Access (LTCA) department performs assessment and intake for all long term services and supports (LTSS) including home and community based services and Medical Assistance-paid nursing home care.
Once a referral call is made through the PCA Helpline an Assessment Worker will complete a face-to-face comprehensive level-of-care (LOC) assessment. The LOC assessment identifies what long term services and supports are most appropriate for the individual aged 18 or older. The assessment takes into account all facets of the person’s life. In addition to reviewing the individual’s medical history the Assessment Worker considers the person’s environment financial resources support systems cognitive functioning and ability to carry out activities of daily living. The Assessment Worker determines the appropriate level of care and provides the individual with an array of options to meet identified needs.
The LTCA assessment is required for the following programs managed by Philadelphia Corporation for Aging (PCA): Long Term Care Program; Caregiver Support Program (CSP); Domiciliary Care Program (Dom Care); and for these programs managed by the Department of Human Services (DHS): Nursing Home Medical Assistance Grant; Personal Care Home; SSI Supplement; Living Independently For the Elderly (LIFE); Attendant Care Waiver; and Independence Waiver.
